Our Vision is that older adults live in their communities of choice as they age; where culture, relationships, supports and services enhance their ability to live independent, active, and engaged lives.

Our Mission is to ensure implementation of an integrated, person-centered, replicable, continuum of supports and services for older adults and those with disabilities as they navigate transitions of aging.

Using a Collective Impact approach, the Continuum is a driver of action through project teams and seizing opportunities that arise. Its’ approach is to leverage community capabilities and resources first, rather than building a new service organization. We will provide leadership and communication in the primary areas of:
  • Housing
  • Information & Referral
  • Linkages (gateway, navigation, wrap-around)
  • Social Connections
  • Policy & Advocacy
  • Incubating tech and other innovations.
  • Data collection evaluation, sharing findings and supporting replication efforts

The Berkeley Continuum is a collaborative partnership (see Leaders and Members) and operates as a project of Ashby Village, Berkeley, CA.

In partnership with City of Berkeley, the Continuum was awarded the SCAN Foundation's Innovation Award for 2019. Click the link here to see a short video.
